Guest blogger Samantha Vitale, DVM, MS, DACVIM (Neurology) from virtual specialists <a href=\"https:\/\/stratocyte.com\/primary-care-veterinarian\" target=\"_blank\" rel=\"noreferrer noopener\">Stratocyte<\/a>, discusses what to consider when imaging the spinal trauma patient. <\/p>\n\n\n\n<h2 class=\"wp-block-heading\">First things first<\/h2>\n\n\n\n<p>When a patient presents with a history suggesting spinal trauma, there are several immediate factors to consider.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>First, assume that there is an unstable spinal fracture or luxation until proven otherwise! Do your best to limit spinal mobility during your initial assessment to prevent further injury.<\/li><li>Equally as important is a thorough assessment of your patient. Unfortunately, traumatic injuries often affect multiple body systems, so ensuring your patient\u2019s overall health and stability is essential.<\/li><li>Once your patient is stabilized, perform an abbreviated neurologic exam. Assess their mentation and cranial nerves to look for signs of head trauma. If your patient is recumbent, do not force them into a standing position. To the best of your ability, assess the movement, sensation, and reflexes in all four limbs to help you determine your patient\u2019s prognosis and guide treatment recommendations. This evaluation should be performed with limited manipulation of the spine.<\/li><li>Finally, as soon as you have had a chance to assess your patient, you should administer analgesia.<\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Consider your options<\/h2>\n\n\n\n<p>There are 3 imaging modalities that can be used to evaluate patients with spinal trauma: radiographs, Computed Tomography (CT), and Magnetic Resonance Imaging (MRI). Let\u2019s take a look at the pros and cons of each one when it comes to small animal spinal trauma.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Radiographs<\/h3>\n\n\n\n<p>Pros:<\/p>\n\n\n\n<ol type=\"1\" class=\"wp-block-list\"><li>Fast!<\/li><li>Anesthesia is not required<\/li><li>Great screening tool for fractures and luxations.<\/li><\/ol>\n\n\n\n<p>Cons:<\/p>\n\n\n\n<ol type=\"1\" class=\"wp-block-list\"><li>Two-dimensional view only<\/li><li>You cannot evaluate the spinal cord itself on radiographs<\/li><li>Radiographs are not as sensitive as CT and MRI for spinal fractures and luxations. Approximately 25% of spinal fractures and luxations can be missed on survey radiographs.<\/li><li>Movement of the patient is often required to obtain 2 orthogonal views<\/li><\/ol>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Computed Tomography (CT)<\/strong><\/h3>\n\n\n\n<p>Pros:<\/p>\n\n\n\n<ol type=\"1\" class=\"wp-block-list\"><li>Fastest option for 3-dimensional imaging<\/li><li>Highest sensitivity for evaluating bone pathology<\/li><li>A reliable method to detect soft tissue trauma<\/li><li>In cases of suspected polytrauma, you can easily CT your whole patient to screen for head, thoracic, and abdominal trauma as well as spinal trauma.<\/li><li>CT is the gold standard for pre-surgical planning when it comes to surgical fixation of spinal trauma, because the high-quality 3D bone imaging allows for measurements and even 3D reconstructions of the images.<\/li><\/ol>\n\n\n\n<p>Cons:<\/p>\n\n\n\n<ol type=\"1\" class=\"wp-block-list\"><li>May require anesthesia or heavy sedation, which can be risky in an unstable patient<\/li><li>While CT provides great detail for many soft tissue structures, you cannot evaluate the spinal cord itself.<\/li><li>Several traumatic conditions can be missed with CT alone, including intervertebral disc extrusion and spinal cord contusion\/hemorrhage.<\/li><\/ol>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Magnetic Resonance Imaging (MRI)<\/strong><\/h3>\n\n\n\n<p>Pros:<\/p>\n\n\n\n<ol type=\"1\" class=\"wp-block-list\"><li>MRI is the gold standard for evaluating the spinal cord, and it is the most sensitive method for diagnosing non-osseous spinal cord injuries.<\/li><li>MRI is ideal for evaluating soft tissue injury and is reasonably sensitive for osseous pathology such as fracture\/luxation.<\/li><\/ol>\n\n\n\n<p>Cons:<\/p>\n\n\n\n<ol type=\"1\" class=\"wp-block-list\"><li>MRI requires general anesthesia and takes the longest of all three methods. This is not ideal for systemically unstable patients.<\/li><li>MRI images alone may not be adequate for surgical planning for spinal fractures.<\/li><\/ol>\n\n\n\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\"><div class=\"wp-block-image\">\n<figure class=\"aligncenter size-full is-resized\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/hallmarq.net\/app\/uploads\/2024\/12\/Spinal-trauma-composite.jpeg\" alt=\"\" class=\"wp-image-11566\" width=\"500\" height=\"400\"\/><figcaption>Figure 1 &#8211; A 12-year-old Cocker spaniel was hit by a car and experienced a fracture of L7 with minimal subluxation.<\/figcaption><\/figure>\n<\/div>\n\n\n<p style=\"font-size:12px\">(A) T2W sagittal images demonstrate ventral compression of the caudal cord and conus secondary to a caudo-dorsal displaced fracture of L7 (arrow).
